A questionnaire designed measure patient satisfaction with cancer care. It is designed to address: 1) satisfaction with care during screening and treatment, 2) challenges of poor and minority individuals receiving cancer-related care, and 3) relevance for evaluation of care for both navigated and un-navigated patients. The questionnaire is usually administered within 3 months of screening and again 3 months after the initiation of treatment.
